Incoming First Year Students (Filipino)
- One (1) Original and One (1) Certified True Copy of BS Transcript of Records; One (1) Certified True Copy of Diploma; General Weighted Average (2.50)
NOTE: Graduating students may submit certification of enrollment and TOR covering up to the first semester of the senior year. - Photocopy of NSO Birth Certificate
- Original and Electronic Copies of NMAT result (40% and above)
- Certificate of Good Moral Character from school and 2 recommendation letters from former college professors
- NBI Clearance (original)
- Medical Examination including laboratory results:
- CBC
- Urinalysis
- Fecalysis
- Anti-HBs
- HBsAG
- Chest X-ray
- Personal Interview
- One (1) passport picture with white background and nametag
- Medical/Dental Clearance from the University Clinic
- Professional Regulation Certificate if any… (for Board Courses)
- One (1) long brown envelope (label it with name and contact details)
Foreign Students
- Certificate of Eligibility for Admission into the Medical Course (to be obtained from CHED Central Office – Manila)
- Photocopy of Passport Bio-page
- Certified True Copy of Transcript of Records for Completed Bachelor’s Degree
- Copy of Undergraduate Diploma or Certificate of Graduation authenticated by the Registrar of the School where graduated
- Notice of Acceptance from the School
- National Medical Admission Test (NMAT) Result
Documents to be Presented to the Dean of School of Medicine
- Certificate of Eligibility for Admission (CEM)
- Original Copy of NMAT Result with at least 40 percentile
- Original Copy of Undergraduate TOR with General Weighted Average (GWA) of 2.50, B or at least 83%
- Certificate of Good Moral Character
- Psychological Exam Result (from the SPUP Guidance Center) with at least AVERAGE result
- Copy of Passport and Student Visa
For Transferees
- Clearance for Transfer (to be obtained from CHED Central Office – Manila)
- Letter of Request / Endorsement Letter
- Notice of Acceptance
- TOR
- Transfer Credentials
- Good Moral Character Certificate
- Photocopy of Passport / ACR
